DESCRIPTION:Free Event \nThis event is for IHP members only. \nThis event will be held in-person at the IHP office. The Care Management Collaborative meeting will take place from 8:00 a.m. to 1:00 p.m. followed by a community resource expo\, featuring transportation and vendors from 1:15 p.m. to 3:00 p.m.  \nPlease plan to arrive early to allow for time for parking\, to retrieve your morning snack\, and to find your seat. A morning snack and lunch will be provided.
