DESCRIPTION:Free Event\nThis event is for IHP members only.\nTarget Audience: The intended audience for the meeting is care managers. Practice managers and billers are encouraged to participate in program update segment of the training. \n \nTopic:\n\nTBD*\n\nObjectives:\n\nReview program updates\, resources\, and data to support patient engagement in care management.\nReview evidence-based guidelines related to HEDIS measures\, including Transition of Care (TCM)\, and review practice level data. Discuss best practices to improve outcomes.\nProvide an overview of BCBM behavioral health care management program. Identify opportunities to collaborate with PCP practice care managers to improve patient outcomes through a cohesive team.\nProvide suicide prevention training and resources to assist Care Managers working with patients having a mental health crisis.\n\n*Topic is To Be Determined and will be updated at a later date. \n \nThis event will be held at the IHP office.
