PCP Team

The objectives for this meeting are to be determined and will be updated as they become available. The target audience is primary care practice team members, providers, practice managers, and care managers.

Team attendance is financially incentivized. Active participation and attendance for a substantial portion of the meeting are required to earn monetary incentive.
