IN-PERSON Care Management Collaborative (CMC)

This event is for IHP members only.

This event will be held in-person at the IHP office. The Care Management Collaborative meeting will take place from 8:00 a.m. to 1:00 p.m. followed by a community resource expo, featuring transportation and vendors from 1:15 p.m. to 3:00 p.m.

Please plan to arrive early to allow for time for parking, to retrieve your morning snack, and to find your seat. A morning snack and lunch will be provided. If you have any dietary restrictions, please contact the IHP office at rainwatera@integratedhealthpartners.net

This nursing continuing professional development activity was approved by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

Target Audience: The intended audience for the meeting is care managers.  Practice managers and billers are encouraged to participate in program update segment of the training.
