IN-PERSON Care Management Collaborative (CMC): Cultural Awareness and Health Literacy

This event is for IHP members only.

Target Audience: The intended audience for the meeting is care managers. Practice managers and billers are encouraged to participate in program update segment of the training.

Topic: Cultural Awareness and Health Literacy

Objectives:

• Practice motivational interviewing skills to foster and engage patients of various cultures and health literacy levels.
• Discuss documentation of care management services and related coding guidelines.
• Discuss the impact of health literacy and how it impacts patients’ health care.
• Discuss the impact of different cultures and how it impacts patients’ health care.
• Review program updates, resources, and data to support patient engagement in care management.
