• Discuss the impact of Asthma/COPD on patients. Review symptoms, treatment guidelines, proper use of inhalers, and programs to support patients and prevent exacerbation of Asthma/COPD.
• Review BCBSM care management program and resources for patients.
• Discuss resources and programs available through Albion Health Care Alliance and identification of opportunities to collaborate with primary care practices.
